ABSTRACT

Objective@#To investigate the clinical and laboratory features of Aeromonas bacteremia in patients with hematological diseases, and provide evidence for the prevention and treatment of Aeromonas infection.@*Methods@#A retrospective study of patients with bloodstream infection of Aeromonas in our hospital from January 2014 to December 2018 was carried out. The clinical characteristics, antimicrobial susceptibility, infection seasons, antimicrobial therapy and evolution were analyzed.@*Results@#A total of 42 patients with hematological diseases had Aeromonas bloodstream infection within 5 years. Among them, 39 cases (92.9%) of bloodstream infection occurred in the stage of neutropenia. The median time of fever was 4 (1-27) d, 22 (52.4%) patients only had fever, 6 (14.3%) with gastrointestinal symptoms (abdominal pain, diarrhea, nausea, upper gastrointestinal bleeding) , 8 (19.0%) with pulmonary infection, 13 (31.0%) with skin and soft tissue infections. Seven patients (16.7%) died with skin and soft tissue infection. The resistance of Aeromonas to carbapenems was 68.3%-70.7%, while the resistance rate to cephalosporins, quinolones and aminoglycosides were less than 10%.@*Conclusions@#Aeromonas bacteremia in patients with hematological diseases mainly occur in the neutropenia stage, usually with symptom like fever. The mortality is increased when accompanied by skin and soft tissue infection. Antibiotic use should be based on susceptibility results, and avoid the use of carbapenems.

ABSTRACT

Objective@#To analyze the hints role of surveillance cultures of Carbapenem-resistant Enterobacteriaceae (CRE) by perianal swabs in patients with hematological diseases, and seek risk factors of CRE bloodstream infection.@*Methods@#The resistance of CRE from 2 914 patients with hematological diseases who cultured perianal swabs, CRE bloodstream infection and risk factors were analyzed during January 2016 to December 2017.@*Results@#In this study, perianal swabs from 2 914 patients with hematological diseases were cultured, 74 patients were CRE positive, and bloodstream infection with CRE was found in 13 of these patients. A total of 87 CRE strains were isolated (The same patient only keep the first one for the same location), including 31 Klebsiella pheuminiae, 43 Escherichia coli, 8 Enterobacter cloacae and 6 other Enterobacteriaceae. The resistance rates to piperacillin / tazobactam, imipenem, meropenam, amikacin, levofloxacin, tigecycline were 91.9%, 74.4%, 98.8%, 17.6%, 74.4% and 8.0%, respectively. Resistance to carbapenem, aminoglycoside, quinolones and tegacycline were highly consistent between two sites from 13 patients, whose both perianal swabs and blood were positive in CRE cultures. Febrile neutropenic time, digestive tract symptoms and perianal infection were independent risk factors for bloodstream infection in patients with perianal swabs positive results, the odds ratios (OR) were 1.10 (P=0.029), 1.13 (P=0.005) and 1.23 (P=0.016), respectively.@*Conclusion@#Perianal swabs surveillance cultures of CRE can be hints for CRE bloodstream infection in patients with hematological diseases, and also can provide suggestions for antibiotics. Long time of febrile neutropenic, digestive tract symptoms and perianal infection can be the early warning for CRE bloodstream infections.

ABSTRACT

Objective:To analyze the results of syphilis specific serological tests among entry personnel in Beijing port during year 201 2 -201 4 in order to provide evidence for the prevention and control of syphilis.Methods:Demographics data and syphilis specific test results of entry personnel were obtained from the information system of Beijing International Travel Healthcare Center and input into Excel 201 6. Database for entry personnel was built up accordingly and was analyzed by IBMSPSS22.0.Results:The number of entry personnel in Beijing port during year 201 2 -201 4 was 1 06 302 person-time.According to the evaluation criteria,71 308 persons were recruited and analyzed,of whom 277 were tested serologi-cally positive and the total prevalence was 0.39% (95% CI:0.34% -0.43%).The prevalence was highest in the persons aged 40 -49 years (0.81 %)and was higher in males (0.50%)than in females (0.23%).The persons from Africa held higher prevalence (1 .27%)than from other five regions.As multivariate logical regression analysis indicated that the risk of syphilis infection was 1 0.38 times more in the persons aged 40 -49 years than in the persons 0.05).Twenty-seven seropositive cases were found within 6 246 entry persons who were tested repeatedly during the research period,9 of which were seroconversion cases so that the cumulative conversion rate was 0.1 4% (9 /6 228).The average conversion time was (6.3 ±3.0)months.There was no statistical significance between the cu-mulative seroconversion rates of the persons with different genders and nationalities (P of Fisher’s exact test >0.05).Conclusion:Serological detection of syphilis for entry personnel in Beijing port and syphi-lis-related health education should be reinforced to reduce the risk of disease importing and spreading, protect susceptible subjects and promote health.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate pathologic and differential diagnostic features of pediatric Burkitt lymphoma (BL).</p><p><b>METHODS</b>A total of 20 cases of pediatric BL were retrospectively reviewed for their clinical and pathologic profiles. Bone marrow aspiration specimens were available in all cases and bone marrow biopsies were available for immunohistochemical study in 18 cases. Flow cytometry study was available in 16 cases. MYC translocation by FISH method was performed in 11 cases.</p><p><b>RESULTS</b>Atypical lymphocytes with cytoplasmic vacuoles were found in bone marrow smears in all 20 cases and peripheral blood films in all 19 available cases. The bone marrow biopsies showed infiltration by uniform medium-sized atypical lymphocytes with multiple small nucleoli but without the starry-sky pattern in all 18 cases. Immunohistochemistry showed the following results in all 18 cases: positive for CD20, PAX-5, CD10, CD34 and TdT, but negative for bcl-2 and CD3 with Ki-67 > 95%.Flow cytometry showed CD19+CD20+CD10+FMC7+CD22+TdT-CD3- in 16 cases, including κ+ in 8 cases, λ+ in 7 cases, and κ-λ- in 1 case. MYC gene rearrangement by FISH was observed in 10 of the 11 cases.</p><p><b>CONCLUSIONS</b>The histopathology of BL is distinct, including atypical lymphocytes with cytoplasmic vacuoles in bone marrow aspirate, lack of starry-sky patternin bone marrow biopsy. Generally, the diagnosis should be made with a combined immunophenotype and FISH approach. Pediatric BL must be distinguished from DLBCL and B-cell lymphoma, unclassifiable, which has intermediate features between DLBCL and Burkitt lymphoma.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the clinical and pathologic features of multiple myeloma(MM) with CCND1.</p><p><b>METHODS</b>Retrospectively analyzed the clinical and pathologic profiles of 158 patients with MM from 2010 to 2013. The clinical and morphologic features of bone marrow aspiration, biopsy and immunophenotypic analysis which was carried out by flow cytometry and immunohistochemistry were analyzed in all patients with MM respectively. CCND1 translocation was studied by FISH method in all cases. Classical cytogenetic studies of bone marrow were performed in 24 cases whose CCND1 was positive.</p><p><b>RESULTS</b>In the 158 patients with MM, CCND1 was detected in 31 patients (19.6%). In 31 patients, type IgA, IgD, IgG, IgM, light-chain only and nonsecretory MM were 4 cases,4 cases,11 cases,1 case, 6 cases and 5 cases respectively. A high incidence of CCND1 was observed in IgD and nonsecretory MM comparied with IgA and IgG respectively (P<0.05). but no statistical significance was reached between κ and λ type patients (P=0.627). The morphology of plasma cell in bone marrow biopsies were small Lymphocyte- Like 24 cases,mature plasma cell 6 cases and immature plasma cell 1 case. Immunophenotype of all 31 cases was CD38⁺CD138⁺CD19⁻CD45⁻, (CD56⁺ in 11 cases, CD20⁺ in 9 cases, CD117⁺ in 3 cases. MM with CCND1 showed a strong association with CD20 expression, the lack of CD56 expression. Immunohistochemistry showed positive for cyclinD1 in 22 cases.</p><p><b>CONCLUSION</b>A high incidence of CCND1 was detected in the IgD and nonsecretory MM, and correlated with Small Lymphocyte- Like, higher positive rate of CD20, cyclinD1 and the lack of CD56 expression. MM with CCND1 must be distinguished from LPL and other mature B cell lymphomas which have plasmacytoid differentiation.</p>

ABSTRACT

Objective To establish a magnetic nanoparticles separation-based quantitative real-time PCR(RT-PCR)assay for fast and accurate detection of Plasmodium falciparum and providing a technical support for improving the control and preven-tion of imported malaria. Methods According to the conserved sequences of the P. falciparum genome 18SrRNA,the species-specific primers and probe were designed and synthetized. The RT-PCR was established by constructing the plasmid standard , fitting the standard curve and using magnetic nanoparticles separation. The sensitivity and specificity of the assay were evaluat-ed. Results The relationship between the threshold cycle(Ct)and logarithm of initial templates copies was linear over a range of 2.5×101 to 2.5×108 copies/μl(R2=0.999). Among 13 subjects of entry frontier,a P. falciparum carrier with low load was de-tected by using the assay and none was detected with the conventional examinations(microscopic examinations and rapid tests). Conclusion This assay shows a high sensitivity in detection of P. falciparum,with rapid and accurate characteristics,and is especially useful in diagnosis of P. falciparum infectors with low parasitaemia at entry-exit frontier ports.

ABSTRACT

Objective To explore the bone marrow pathology ,diagnosis and differential diagnosis of Waldenstrom macroglobulinemia(WM). Methods 19 WM patients was examined by bone marrow aspiration (BMA) and bone marrow biopsy (BMB) for morphology. Flow cytometry (FCM) and immunohistochemistry (IHC) for immunophenotyping. Results Plasmacytoid lymphocytes were identified in 11 BMA. All of 19 BMB were involved by lymphoma cells. 17 cases showed a predominance of small lymphocytes and 2 of plasmacytoid lymphocytes. Typically plasmacytoid lymphocytes were not seen in 4 cases. Patterns of bone marrow involvement were as follow: diffuse (12 cases), nodular (4 cases), interstitial (3 cases). Immunophenotypically, FCM showed all cases were CD_(19)~+, CD_(20)~+, CD_(22)~+, CD_5~- and CD_(10)~-. IHC revealed small lymphocytes and plasmacytoid lymphocytes were Pax5~+ CD_(20)~+ and plasma cells were CD_(38) CD_(138)~+ CD_(20)~- Pax5~-. Conclusion Small lymphocytes proliferation with plasmacytic differentiation is the typical bone marrow pathologic features of WM. IHC is benefit for identifying lymphocytes and plasma cells components. The Combination of morphology, FCM and IHC is contributive to the diagnosis and differentiation of WM.

Obiective:To investigate the effect of interleukin-6(IL-6)on apolipoprotein(apo)M expression in a human hepatoblastoma cell line(HepG2).Methods:HepG2 cells were cultured and incubated with different concentrations of IL-6 (0,1.25,2.5,5,10,20,40 and 80 μg/L)for 24 hours.After the incubations,total RNAs were extracted and applied to reverse transcript PCR and real-time quantitative PCR to detect the expression levels of apoM and apoA-I.The effect of IL-1α on apoM expression was also determined.Results:IL-6 significantly inhibited the expression of apoM(F=10.778,P < 0.01).Whereas IL-6 did not influence the expression of apoA-I(F=2.004,P > 0.05).IL-1α(0,1.25,2.5,5,10,20,40 and 80 μg/L)did not decrease the expression of apoM(F=2.038,P > 0.05).Cooclusion:IL-6 inhibits apoM mRNA transcription,which may contribute to the pathogenesis of abnormal lipid metabolism and macrovascular complications in type 2 diabetes.

ABSTRACT

In the process of designing genechip probe for detecting pathogenic microorganism, the selection of single nucleotide polymorphism (SNP) combination is of great importance. At present, there is no automatic design method. This work is hard and the result is not always well. A new approach for selecting SNP combination is presented in this paper. Genetic algorithm is used to search optimal solution on the basis of classification ability of SNP combination, which is evaluated by the rough set theory. Other related experimental parameters are also incorporated. Experimental results show that the method can find out the best SNP combination pattern efficiently and accurately, thus demonstrating the reliability of this new method for designing the genechip probe.

ABSTRACT

AIM: To examine the pharmacodynamics of a self-developed oral colon-specific delivery coated tablets of 4-aminosalicylic acid (4-ASA) on rats. METHODS: Ulcerative colitis rat model was developed by intrarectal administration of 2, 4, 6-trinitrobenzenesulfonic acid (TNBS) in alcohol. Rats were divided randomly into healthy control group, TNBS control group, TNBS + lower dose (coated tablets) group, TNBS +medium dose (coated tablets) group, TNBS + higher dose (coated tablets) group, TNBS + sulfasalazine (SASP) group, TNBS + medium dose non-coated tablet group. Several indices including macroscopic change,histological damage, and tissue myeloperoxidase (MPO) activity were examined after 5 consecutive oral drug administration to assess pharmacodynamics of the coated tablets. RESULTS: An experimental ulcerative colitis in rats was induced by TNBS. Compared with 4-ASA non-coated tablet group, decreased macroscopic and histological damage score and lower MPO activity were observed after the oral administration of 4-ASA coated tablets or SASP. There was no significant difference between the effects on the indices of higher dose of coated tablets and SASP (P > 0.05) CONCLUSION: The self-made 4-ASA oral colon-specific delivery coated tablet showes higher effect than non-coated tablets to treat ulcerative colitis in rats.

ABSTRACT

Objective: To observe the effect of 70% ethanol extract from Huanglian Jiedu Decoction on expression of S180 tumor cell apoptosis factor in MDR model mice, and to research the molecule biology base fot directing clinic. Methods: simulate Clinic PFC project was simulated, the mice model of multi-drug resistance of S180 tumour cell was established. 70% Ethanol extract from Huanglian Jiedu Decoction were given for 10 days. The flow cytometry was used to observe Fas, Trail, CD54.,et al. Resluts: 70% Ethanol extract from Huanglian Jiedu Decoction can increase expression rate of Fas, Trail and improve apoptosis of S180 cell, at the same time it can decrease expression of CD54.Conclusions: 70% Ethanol extract from Huanglian Jiedu Decoction improve high level expression of apoptosis factor, and it maybe one of pass reverse multi-drug resistance of tumour.

ABSTRACT

AIM:To establish the method of determining sodium new houttuyfonate in plasma and study its pharmacokinetics in beagle dogs.METHODS:The HPLC was employed with chromatographic conditions,such as octa-silicane bond silica gel column(250 mm?4.6 mm,5 ?m)as analytical column,methanol-water-ammonium hydroxide with 4 butyls(70 ∶30 ∶0.3)as mobile phase and at the flow-rate of 1.0 mL/min.The UV detection was set at 286 nm.The pharmacokinetic parameters were calculated by DAS(ver 1.0)program and compared by statistic analysis.RESULTS:The calibration curve was linear in blood plasma over the concentration range of 0.128~25.575 mg/L(r= 0.999 2),good precision and high recoveries.The parameters were T_ max= 5.0 min,C_ max= 4.982 mg/L,T_ 1/2=8.67 min.CONCLUSION:The analytical method established in this study is sensitive and exact,and can be used to measure the pharmacokinetics of sodium new houttuyfonate in beagle dog.The sodium new houttuyfonate can metabolize quickly following intravenous injection.

AIM: To observe the relationship between the reversal effect of 4 alkaloids of TCM on expression of P170 being obtained multi-drug resistance to mouse S180 tumor cell induced by chemotherapy protocol of PFC and the adjustment of the cell apoptosis.And to discuss the difference among alkaloids of TCM on reversion of being obtained multi-drug resistance in clinic.To improve the curative effect thereby. METHODS: By the methods of less dosage of Chemotherapy PFC,to set up the mouse models of multi-drug resistance of S180 tumor cell.At the same time to give the mouse matrine,terandrine,oxymatrine and berberine hydrooh loride for 2 weeks and then to observe the P170,TOPOⅡ,Fas and apoposis by flow cytometry. RESULTS: Matrine and terandrine could obviously reduce the expression of P170,the activation of TOPOⅡ induced by Chemotherapy and increase the ratio of the express of Fas and the apopsis of drug resistance to tumor cell.And berberine hydrochloride could obviously reduce the expression of TOPOⅡ. CONCLUSION: The adjustment to the apoposis of tumor cell by matrine and terandrine is related to the reversion of being obtained multi-drug resistance to tumor cell' expression,besides the different degree
